She was warm to the touch then and but 6:30 am had a fever of 99.6. Within 45 minutes her face was swollen, she had a red rash all over her belly and her fever had jumped to 101. Dr. C was there and saw her rash. Based on her presentation and the fact she was quite itchy overnight, he thought it was a reaction to the plasmapheresis last night. Benadryl and Tylenol were given and she improved. Swelling went away, fever broke briefly, and her rash improved but never went away. She was up playing with PT and OT the rest of the morning. About 11ish, Ultrasound came to do her liver ultrasound and to do one on her centra lines to look for a clot. Sure enough they found a clot on her HD line. We were far from impressed with the resident today. She came in and said there was a clot. So two hours later as Nora is getting for febrile, more swollen, and more fussy, the nurse mentions it was a DVT so she doesn&#8217;t want to use the line due to the risk of the clot moving! That was not relayed!!! Ugh! Also it should not have been us checking Nora&#8217;s temperature every hour. Nora got more Tylenol at noon, but from noon to 3 her temperature only climbed and her swelling and redness around her incision worsened. So at 3, her temperature was 103.5 axillary and the nurse called a rapid response, which means the ICU code team comes up quick. When they got there Nora was laying on the bed pretty listless (which we all know is not her at all). Her temperature at that time was 104.2! So her port was accessed but unable to draw from due to a clot. We were taken back down to the PICU. Dr. C met us down in the PICU at about 6 to remove her HD line as they ended up removing her JP drain as well. Both had clots and they were concerned for infection in them. They have taken so many blood cultures today with the new fever with no results yet. We should know more tomorrow. Dr. C and the PICU attending are pretty positive she has a drug reaction and\/or a reaction to the plasmapheresis last night. Dr. C said this is not uncommon and he was not overly concerned. They are acting on it as quickly as possible and doing everything they can to treat it. They have changed up her antibiotics and have started her on heparin to help break up the clot. So for now we will try to get some rest in the PICU. Since we have been here, Nora is doing better and her heart rate and fever have come down a lot!
