Volunteer Supply Order Form

The Children’s Organ Transplant Association (COTA) offers materials at no cost to COTA campaign teams to be used for fundraising purposes.

Processing – Allow two to three business days to process and pack an order. If materials are not in stock, you will be contacted regarding projected delivery date or quantity available to partially fill an order.
Shipping – Orders will be shipped via postal service or UPS ground service. You will receive an email confirmation from COTA with tracking information. Please allow an additional 5 to 7 business days for shipping.
