Este post va a estar largo entonces necesito que se sienten.
La recuperación de Vale tomó un rumbo MUY inesperado que empezó hace 10 días.
Una de las válvulas del corazón de Berlín no estaba bombeando bien entonces le hicieron una eco cardiaca para ver si había algún bloqueo en alguna de las canulas que están en el pecho. Para sorpresa de todos encontraron que la función del corazón de Vale había mejorado sustancialmente y su propio corazón estaba trabajando por encima del corazón de Berlín. Para que se hagan una idea la forma como se evalúa la función del corazón (o la más determinante) es la fracción de eyección que los rangos normales deben ser más de 55%. Cuando a Vale la admitieron hace casi 8 meses su fracción de eyección era 8% y ahora está en 60%!!!.
Esto es EXTREMADAMENTE inusual y hay MUY pocos casos en el mundo de niños que hayan mostrado una recuperación tan significativa sobre todo considerando como estaba de enfermo el corazón de Vale. Es la primera vez que pasa en este hospital. Médicamente hablando no encuentro una explicación muy lógica para lo que está pasando y yo personalmente pienso que es en parte el tratamiento médico que ha tenido desde el principio dándole reposo al corazón y medicamentos para falla cardiaca, en parte ha sido también estar aquí con la bebe todo el tiempo y contribuir con su desarrollo y en gran parte tiene que ser un milagro.
El plan es ahora entonces tratar de sacarla del corazón de Berlín poco a poco para ver cómo responde el corazón cuando esté trabajando solo.
Los últimos 3 días le han hecho pruebas disminuyendo la ayuda del corazón de Berlín y le ha ido MUY bien.
Hoy nos reunimos con el cirujano y el cardiólogo y revisando exámenes pasados se encontraron que la coartación de la aorta que le diagnosticaron al principio (que fue la causa de todo esto) y que ya le operaron, está otra vez un poco estrecha entonces la van a hacer un cateterismo posiblemente esta semana para dilatarle ese pedazo de la aorta antes de seguir con las pruebas finales antes de pensar en sacarle el corazón de Berlín.
El proceso de sacar el corazón de Berlín también tiene sus riesgos que implican principalmente la formación de coágulos porque al parar el corazón de Berlín la sangre se estanca, entonces le tienen que subir mucho al anticoagulante mientras hacen las pruebas.
Obviamente también existe la posibilidad que no pase alguna de las pruebas finales y volveríamos a donde estábamos que es seguir esperando el trasplante pero los médicos y el equipo de trasplante están muy optimistas y después de hablar con ellos hoy nos sentimos más cómodos compartiendo la información con ustedes (no queríamos generar mucha ansiedad).
Si a Vale la logran sacar del Corazón de Berlín NO necesitaría el trasplante.
Estamos hablando de un proceso que se demoraría más o menos 7-10 días.

Vale’s recovery took a VERY unexpected route about 10 days ago.
One of the Berlin heart’s valves started malfunctioning (very low ejection) so everybody got concern and they decided to do an echocardiogram to check on the cannulas in her chest and make sure they were not blocked.
Surprisingly they found that Vale’s native heart function was way better than before and her own heart was actually overriding the Berlin heart.
Just so you have an idea of how this works, the heart function is measured (mostly) through the percentage of blood ejection called the ejection fraction and the normal value should be over 55%. When Vale was admitted to the hospital about 8 months ago her ejection fraction was 8% and the last echocardiogram showed a 60%!!!.
This is VERY unusual considering how sick she was and there are very few cases documented worldwide (it’s also the first time they see a recovery like this in this hospital).
Medically I can’t really find any logical explanation for this other than this was probably a combination of great medical management of her heart failure and resting the heart for a while, great family (and friends) support but most likely a miracle as well.
So the plan now is to slowly wean her off the Berlin heart to check on her heart’s response on its own.
They’ve been doing weaning trials for the last 3 days and she continues to do great.
We met with the surgeon and the cardiologist today and they reviewed all her prior tests again and noticed that there’s still some narrowing on her aorta (remember this is what started everything and she already had a surgery to correct it) so they’re planning to do a heart catheterization probably this week to balloon that area and open it up prior to continuing with her weaning trials.
The weaning off process is a little complicated and takes somewhere from a 7-10 days and involves some risks (mostly blood clots in the Berlin valves) so she needs to be on higher dose of anticoagulants.
There’s obviously the possibility of her failing the trials and going back to where we were before (transplant) but the transplant team seemed very optimistic so we felt comfortable delivering this information. We didn’t want to create a lot on anxiety.
If they are able to wean her off the Berlin heart most likely she WON’T need a transplant.
